Core Integrative Services: Client Agreement

1. Payment Terms & Financial Policy

  • Total Investment: The total cost for the 4-month program is $1,196.00.

  • Payment Schedule: Payments are due [e.g., in full upfront with 10% discount / in four monthly installments of $299.00/mo]. If on a payment plan, the first payment is due upon signing, and subsequent payments will be automatically charged every 30 days.

  • Invoicing: All invoicing and payment processing are handled through the Healthie and Stripe platforms.

  • Accepted Methods: We accept all major credit cards and debit cards and in some cases, HSA/FSA cards. We do not accept insurance or personal checks.

  • Late Payments: If a payment is more than 3 days late, all services, lab reviews, and consultations will be suspended. If a payment is 15 days late, the account will be charged a late fee of $50, and will continue to be charged $50 every 15 days until the payment is completed. 

  • End of Relationship Payments: If the relationship ends early, the "Prorated Professional Service Refund" policy (as outlined in our Refund Policy) applies. Any outstanding balance for services already rendered will be charged immediately to the card on file.

2. Mutual Responsibilities & Expectations

Both parties agree to the following:

  • Consultant Responsibilities: We agree to provide high-level services and functional lab support, educational resources, and lifestyle recommendations. We agree to respond to HIPAA-compliant messages within 24-48 business hours.

  • Client Responsibilities: You agree to:

    • Complete all intake forms and assigned materials at least 12 hours prior to scheduled calls.

    • Provide honest and accurate information regarding your health history and current concerns.

    • Actively participate in the coaching process and implement agreed-upon recommendations.

    • Maintain respectful and professional communication throughout the coaching relationship.

3. Termination of Agreement

  • Either party may terminate this agreement at any time with fourteen (14) days’ written notice by sending an email to info@coreintegrativehealth.co.

  • Termination by Consultant: We reserve the right to terminate this agreement immediately if the Client is consistently non-compliant with protocols, misses multiple appointments without notice, or acts in a way that is abusive or unprofessional.

  • Effect of Termination: Upon termination, the client is entitled to a prorated refund of unearned fees (see Section 2 of Refund Policy), but is not entitled to further access to the Healthie portal or proprietary educational materials.

4. Confidentiality & Privacy

We value the trust you place in us. All information shared during sessions, within the Healthie portal, or in lab results is considered Confidential Information.

We will not disclose your identity or health data to any third party without your express written consent, except as required by law or for the purpose of processing lab tests with our third-party partners.

B. Proprietary Materials: You agree to keep all Core Integrative Services proprietary protocols, guides, and educational materials confidential and will not share, record, or distribute them to third parties.

C. Media & Testimonial Release: We love celebrating your wins! By signing this agreement, you grant Core Integrative Services permission to share screenshots or quotes of positive results or feedback shared by you in the Healthie portal or via electronic communication for marketing and educational purposes (e.g., Instagram, website, or newsletters).

  • Privacy Guarantee: Before sharing, we will strictly redact all Personally Identifiable Information (PII) and Protected Health Information (PHI), including your name, profile picture, specific dates, and any other unique identifiers, to ensure your identity remains 100% anonymous.

  • Right to Opt-Out: If you prefer that your messages never be shared, even anonymously, please check the box below or notify us in writing at any time to revoke this permission.

5. Professional Disclaimer & Limitation of Liability

  • Not Medical Advice: Core Integrative Services, LLC provides wellness and nutritional education only. We are not licensed physicians or mental health professionals. Our services do not replace the care of a primary care provider. Do not disregard professional medical advice or delay seeking it because of something you have read or discussed in this program. We encourage always consulting your licensed healthcare provider before making changes to your health routine.

  • No Guarantees: You acknowledge that health outcomes are highly individual. Core Integrative Services, LLC does not guarantee results, improvements, or cures.

  • Release of Claims: To the maximum extent permitted by law, you agree to release Core Integrative Services, LLC, its owners, and employees from any and all liability, damages, or causes of action arising out of your participation in this program. Your implementation of any suggestions is done solely at your own risk.
