Program Terms, Conditions, & Refund Policy
1. Lab Testing Fees
Refunds for functional lab tests are handled in accordance with the specific refund policies of the respective third-party laboratory testing companies. Once a lab kit has been shipped or registered, any eligibility for a refund is determined solely by the testing company's terms. CORE Integrative Services, LLC is not responsible for any laboratory-specific processing fees, shipping costs, or non-refundable balances as dictated by the lab provider.
2. Prorated Professional Service Refunds
If a Client chooses to cancel their 4-month program for any reason, a prorated refund of the Professional Service Fee will be issued based on the following:
Completed Services: Any consultations held, intake reviews performed, or lab interpretations delivered will be billed at our standard monthly rate from the program total.
Remaining Balance: Any unearned portion of the professional fee will be returned to the original payment method.
3. Appointment Cancellation & No-Show Policy
We require notice for all appointment changes:
Cancellation Notice: Clients must provide at least 24 hours' notice to reschedule or cancel a consultation.
Late Cancellations/No-Shows: Appointments cancelled with less than 24 hours' notice, or missed entirely without notice, will be handled on a case-by-case basis. At the sole discretion of CORE Integrative Services, LLC.
4. Discretionary "Pause" Option
In the event of significant life changes (such as pregnancy or medical emergency), a program pause may be granted on a case-by-case basis at the sole discretion of CORE Integrative Services, LLC.
Duration: Pauses are generally limited to 3 months.
Re-Entry: Because health data and symptoms change over time, pauses exceeding this window may require the Client to undergo a new intake process and updated lab testing at the Client's expense to ensure protocol safety and accuracy.
5. No Results Protection & Client Compliance
No Guarantees: We provide wellness education and health coaching services. Individual results vary based on personal factors and level of engagement, and specific results cannot be guaranteed. Refunds are not issued based on subjective dissatisfaction or perceived lack of progress.
Expiration: All labs and consultations must be completed within 6 months of purchase or they will be forfeited.
Non-Compliance: We reserve the right to terminate the relationship without a refund if a Client consistently fails to provide required information or adhere to the agreed-upon protocol.
6. Chargeback Protection
Client agrees to follow the refund procedures outlined herein and shall not initiate a credit card chargeback for services already rendered. Client will be responsible for all legal and collection fees associated with disputing unauthorized chargebacks.
California SB-577 Disclosure
1. Nature of Services: We are not licensed physicians or surgeons. We are not licensed by the State of California as healing arts practitioners. We do not diagnose, treat, cure, or prevent any disease, physical or mental condition, or injury. Our services are alternative or complementary to healing arts services licensed by the State of California.
2. Theory of Treatment: We provide functional lab interpretation for educational purposes only. This information provides insights into your nutritional and physiological status and is not a substitute for medical advice. Recommendations focus on supporting the body’s natural systems through nutrition and lifestyle adjustments.
3. Qualifications: We hold the Certified Nutrition Practitioner (CN-P) credential through Kale Health and the Integrative Health Practitioner (IHP) credential through the Integrative Health Practitioner Institute (IHPI).
Month-to-Month Service Terms
Applicable upon completion of the initial 4-month program.
Automatic Renewal: Month-to-month services will automatically renew every 30 days.
14-Day Cancellation Notice: To cancel services, Clients must provide written notice at least fourteen (14) days prior to their next billing date.
No Partial Refunds: Once a monthly payment has been processed, no partial refunds will be issued for that cycle; services remain available until the end of the final billing period.
